FINANCIAL POLICY


Payment is due at time of service which includes deductibles and
estimated copays
.

We offer an extended payment plan with prior credit approval
through Care Credit for up to twelve months with no interest
depending on amount financed.

We offer a 5% discount for payment made in full at the time of
service over $500.00.

For services that require more than one office visit the payment can
be divided among the number of visits needed.

Insurance filing is done as a courtesy to our patients.  We will do our
best to estimate the patient's copay but in the event there is a
difference the responsibility for payment belongs to the patient.
We make every effort to schedule our patients promptly and according
to their needs.  Appointment hours start at 8:00 am and our office
closes at 5:00 pm.  We do not schedule between 12:00pm and
1:00pm, and we are closed on Fridays.

You will be provided with a self adhesive appointment card indicating
the time and date of the appointment you made upon leaving our office.

On all appointments made six months in advance we will send out a
card by mail at least two weeks prior to your appointment.  If you find
that this is not a good time please call as soon as possible to
reschedule.  We will provide you with a courtesy call the day before
your scheduled appointment.

We request that if you cannot keep an appointment please allow at
least 24-48 hours notice so we may extend that time to another patient.

Our office works hard to maintain a timely and efficient schedule so
that we can better serve our patients.  We ask that you arrive on time
as to assist us in keeping wait time to a minimum.  If you see you may
be late please call our office to inform us of the situation.
